  • About Helen Porter Rehabilitation and Nursing

    Our dedicated, experienced nursing team cares for Helen Porter residents 24 hours a day. With 98 beds, Nurse Practitioners, Registered Nurses, Licensed Practical Nurses and Licensed Nurse Assistants provide daily clinical needs, including personal hygiene, safety and comfort. We tailor our approach to each resident, always providing compassionate, respectful care. Helen Porter offers, long term care, short term rehabilitation, and memory care units.

    Champlain Valley Physicians Hospital Skilled Nursing Facility is a 34-bed nursing home offering round-the-clock care, a full range of medical services and a subacute rehab program. We provide high-quality health care while also supporting each resident’s mental, emotional and spiritual needs.

Jerilyn, Charge RN — Helen Porter

With nearly 25 years dedicated to Porter and her entire career rooted at Helen Porter, Jerilyn has grown from working as a geriatric aide in her teen years — helping with small but meaningful tasks—to a Licensed Nursing Assistant for a decade, and now serves as the night charge nurse in the memory care unit, a role she has held since 2016. Inspired by her mother’s own time in the field, she found her calling early and never looked back, even working evening shifts as an LNA while completing college in 2014. Her experience spans long-term and hospice care, but it’s the relationships that matter most: “I like the consistency by seeing the same residents over and over, because I feel like I can help them better,” she says. And even in memory care, where recognition can fade, connection remains—“They may not know my name, but they know my face and I can help them.”

Michelle, LNA — Helen Porter

Michelle has been an LNA for almost two years, beginning her journey through a paid, accelerated trainee program directly through Helen Porter, that she describes as fast-paced and led by highly knowledgeable instructors. She works primarily on the rehab unit and also picks up per diem shifts on the med-surg floor at Porter Medical Center, gaining a wide range of experience. Before healthcare, Michelle worked in special education, where she discovered her passion for caregiving but felt called to make a broader impact with adult patients. “I enjoy seeing the results in patient care and making patients happy—making a difference in patients’ lives,” she shares. That compassion drives her daily work: “I love making my patients happy, because some patients don’t have people to care for them like we do.” She values the supportive atmosphere and positive culture of her workplace and says her favorite part of Vermont is its close-knit, small community feel.

Diego, LNA — Woodridge

Diego has been working as an LNA at UVM since May, starting his journey through the organization’s training program. He currently works on the long-term living unit, where he’s found a meaningful connection to patient care. Originally interested in physical therapy, Diego shifted toward healthcare after working as an EMT, which deepened his passion and led him to pursue a future in nursing through an LNA-to-RN pathway. What stands out most to him about long-term care is the sense of home and connection: “You’re not having people in and out… it is like a family there, and you really get to know the patients." For Diego, the most rewarding part of each day is simple but powerful—“Interacting with the residents and being able to make them smile and show that you care.” He describes his team as hardworking and empathetic, always ready to support one another, and in his free time, he enjoys the outdoors, especially hiking, snowboarding, and skiing.

Nikki, LPN — Helen Porter

Nikki has spent the past four years growing her career at UVM Health, starting in patient access before completing her LNA training at Helen Porter and earning her LPN last year. Since July, she’s worked as a unit LPN, primarily in long-term care while also supporting rehab and memory care residents. With nearly 20 years in healthcare, nursing was always her goal. She values the consistency of her unit and the relationships she’s built with residents, sharing, “The residents are some of my favorite people.” While the work can be demanding, she finds it deeply rewarding and encourages others to consider the field for the meaningful connections and life lessons it offers. Outside of work, she enjoys Vermont’s summer and fall, especially kayaking, paddleboarding, and fishing.
